Sample COVID-19 Waiver
ACKNOWLEDGEMENT OF ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND AGREEMENT TO HOLD HARMLESS AND INDEMNIFY.
In consideration of being allowed to participate in the Southwest International Folk Dance Institute's (known as and hereafter referred to as SWIFDI) 2022 New Mexico "August" Camp program and related events and activities:
The undersigned understands, appreciates and acknowledges the existence of COVID-19 in the general population, and I acknowledge the risks associated with COVID-19, including serious illness/death which can be spread through person-to-person contact in social activities such as those sponsored by SWIFDI, and I voluntarily undertake that risk in order to participate in SWIFDI's 2022 New Mexico Camp, and by signing below I waive (give up) any and all claims against SWIFDI for damages, liabilities, illness, injuries or death associated with contracting COVID-19 as a result of my participation in SWIFDI.
I further agree to exempt SWIFDI and New Mexico Tech (NM Tech) from any responsibility for damages, liabilities, illness, injuries or death associated with contracting COVID-19 should I suffer illness and/or death as a result of contracting COVID-19 from participating in SWIFDI activities at New Mexico Camp, and I further agree to indemnify and hold harmless SWIFD and NM Tech from any and all claims, liabilities, injuries, or damages resulting from my participation.
This waiver and hold harmless agreement includes and applies to any claims brought by any and all persons who may bring a claim on my behalf.
I agree to present proof of vaccination and negative recent test upon arrival at SWIFDI, and to answer questions about the current state of my health at the time of arrival at SWIFDI or at any other time during camp, and to the taking of my temperature or a rapid test if requested. Masks and social distancing will be at the discretion of the participant except as noted elsewhere, subject to change. Individual rooms may be requested in advance by participants in SWIFDI.
This Agreement includes the entirety of the Agreement regarding COVID-19 between the undersigned and SWIFDI, and precludes the assertion of any other statements regarding COVID-19, written or oral, between SWFDI’s representatives and the undersigned.
